Cultural Beliefs

V.P. is a 4 year old boy who came to clinic last week because his father thought he had a bug in his ear. On arrival the boy was seizing. His oxygen saturation was very low at 69%. His father said he started seizing while on their way to clinic. They live in the mountains about 3 hours from Cazale. His father wasn’t sure how long he had been seizing except that it had started on the way to clinic. The boy was started on oxygen and IV was placed in anticipation of giving him medication to stop the seizure but it resolved spontaneously. The patient was unresponsive for about 30 minutes and then slowly started to wake up. His father said that V.P. started having seizures when he was a baby and that he has at least 1 seizure per month. Each seizure lasts about 30 minutes. The patient has been to our clinic multiple times, but neither of his parents ever mentioned that he has seizures. His father explained that his son’s seizure disorder is treated by a doctor near their house. However, there are not any clinics anywhere near their house. His father was very reluctant to talk to us about the treating physician. Eventually we determined that V.P. takes non pharmacologic medication, so either a natural medication or something prepared by a voodoo priest. It is not uncommon for Haitians to believe that an illness or symptoms they have are caused by spiritual forces or curses.  His father agreed to let us start an anti epileptic medication after extensive education about the negative health consequences of seizures. I am not sure that the patient’s father is convinced that his son’s seizure disorder can be treated with medication. V.P.’s seizures are life threatening and can result in hypoxic brain injury. Please pray for V.P’s parents that they give him the medication and bring him to his follow up appointment.
